Becoming a Patient or Referring a Patient

While Health Centers does its best to accommodate all persons who want to come to be treated at we are unable to care for several types of patients.

We are "Opted Out" of Medicare/Medicaid. This means we can see you but you will have to pay out of pocket and sign a Medicare treatment agreement with Health Centers of America. "

Also, we are not equipped to help anyone who is bedridden or who could not sit up, either in a chair or wheelchair. Therefore, if you cannot enter our building on your own (either by walking, in a wheelchair, walker or cane) then we are unable to help you. If you need assistance with a wheelchair or walker please have a caretaker with you at all times. We are also unable to provide help for patients to use our bathroom facilities, so you would need a caretaker here at all times to help.

ONCE YOU HAVE FILLED OUT THE FORM, SIMPLY HIT THE SEND BUTTON AT THE BOTTOM OF PAGE AND YOUR FORM WILL BE SENT TO HEALTH CENTERS.

When the appointment is set, we will send you a packet of info to fill out. Bring your medical records with you. Out of town patients, the intake packet will include hotel arrangement.

If you need a consult prior to your initial appointment or are requesting a consult only, you will need to fill out the application. Payment arrangements need to be made prior to the phone appointment. Please note that Dr. Ryser cannot and will not give any medical advice or recommendations without having first seen you as a patient.

Appointment, Billing and Fee Information

As a courtesy to our patients, we will bill your insurance carrier for payable services on your behalf. However, as we (Dr. Ryser ), have no contractual arrangements (not a participating provider), we require you pay for your initial visit(s), all initial services and treatment. Any reimbursement from your carrier will be made on a “out of network provider” basis. Most insurance carriers will send any reimbursement to us and you will be responsible for any balances insurance does not cover. Any credit balance that appears on your account resulting from your payments and insurance reimbursement, will be returned to you. We will bill your insurance carrier for all charges. The cost of the initial visit is $450.00. For out of town patients, this means that all the initial visits must be paid for at time of service and amounts to between $700 - $1,000 for the 2- 3 day initial visits.

Co-Pay: Some insurance carriers reimburse at a reasonable rate for “out of network” medical services. You will need to contact your insurance carrier if you are interested in what their reimbursement policy is.

Blue Cross/Blue Shield and Medicare: Since Dr. Ryser is out of network for BC/BS and Medicare, and since neither will send any reimbursement for treatment and services to our office, BS/BC and Medicare patients will be required to pay for all treatment and services at time of service.

Laboratory Testing: Drawing blood for lab testing is performed by our in-house lab and sent to reference labs or specialty labs for evaluation. There will be extra charges for some of the lab work as well as charges for shipping and handling. There is a charge for the blood draws which must be paid for at time of service. Insurance may or may not pay for lab work. IF they do not, YOU WILL BE RESPONSIBLE for the lab charges. Some labs require payment at time of draw to be sent with samples, Some tests insurance won’t pay for. As we have no business relationship with any laboratories, we do not bill or collect for outside labs nor can we negotiate any rates with the laboratories. As a courtesy to our patients, we will send your payment to the lab with your lab draw for those labs that do not take insurance.

Cancellation policy: We require you to cancel your scheduled appointment 24 hours in advance or be charged for that appointment. For Monday's appointments, they must be cancelled by 3:00 pm on the preceding Friday. All initial appointments must be cancelled at least seven (7) days prior to an appointment or you will be charged $150 for time lost Should you have any questions, please call Michael or Rachelle, 816-763-9165 for more information.
